Unintentional surface breech after torpedo launch

Hello again crewmates, I have more questions and I would value your opinions:
When U331 unloaded on the Barham the reports confirm that the sudden weight loss added to the buoyancy of the boat, causing the Uboat to surface right in the middle of the flotilla (a suicidal action?).
This sounds like negligence or/and incompetence on behalf of the submariners:-
What should have been the S.O.P.?
Was this a common occurrence?
Who do you think was to blame?
Discuss? :-)
