Polish SLAZAK (L 26). Built by Hawthorn-Leslie; laid down 25 May 1940, launched 23 July 1941, completed 30 April 1942, commissioned 9 May 1942, Channel escort. 18-19 August 1942: participated in Dieppe landing operation; later again escort service in the Channel; 19.05.1943 left Greenock with KMS.15; since 25 May escort service in Mediterranean; July 1943: artillery support during Sicily landing operation; later escort duties in Med; September 1943, Salerno landing, artillery support and AA cover, later escort duties in Med, 10 April 1944 left Gibraltar for UK, later escort duties in the Channel and preparations for D-Day, 6 June 1944 Normandy, Sword Beach - artillery support, since September 1944 escort service in eastern part of Channel, July 1946 in reserve (Harwich), 28 September 1946 returned to RN.
During Polish service she covered 104 000 miles and escorted 104 convoys.
