A very interesting posting - many thanks.
It brought to mind the loss of HMS/M THETIS in on 1st June 1939. The loss was due to a small drop of enamel (paint)blocking the drain tube on the inner door of number five torpedo tube when her bow tube was open to the sea.
Ninety nine brave men lost their lives that day.
Anyone interested in further information - just put HMS THETIS into a search engine and have a look
