A German World War II submarine was sunk due to malfunction of the toilet.

U-1206 [uboat.net] was sailing out of Kristiansand, Norway, being part of the 11th Flotilla, cruising at a depth of about 200 feet. It was at this point that Captain-Lieutenant Karl-Adolf Schlitt [uboat.net] decided to attend to a call of nature, in a new type of toilet designed for helping at greater depths.U-1206
And similar to lots of new technology, it was a pain in the you know what which gave Schlitt trouble operating it. He decided to seek the help of an engineer who came in and tried to fix it. However, he opened the wrong valve and seawater started trickling into the ship. After the water reached the batteries situated just beneath the toilet, chlorine gas started permeating into the boat and Schlitt was forced to order a resurfacing of the U-1206.
It was not their day, the Germans. Alas! The boat just happened to be 10 miles off the Scottish coast and the British quickly spotted it. The rest is history.
An aircraft suddenly appeared and began a barrage of attack that left four men on deck dead and the boat was so damaged that it was unable to dive, despite attempts by the crew to blow clean air into the boat. It was then that Schlitt gave the order to ‘Abandon ship!’
