The Talisman

Hello,
Would you know of the existence of a Court of Enquiry report into the loss of the Talisman submarine? My father, who lost his father on the Talisman, thinks he remembers reading about such a report being published. So far I've had no luck in tracing it. I've tried the PRO and the Royal Naval Museum.

I have been told that during war there wouldn't have been time to produce such a report but my father is adamant.
