Hi Ron,

the first description is the right one, BALFOUR was hit by NIDD (see file ADM 137-3458 at TNA).

The U-Boat involved was UB 74 which didn't return, but incident was cofirmed by her logbook which was recovered by divers and survived at TNA (see file ADM 137-3900).

UB 74 suffered only minor damage.
