Actually the discussion was about U-boats NOT machine gunning survivors. The Eck case is the only proven case during the entire war of a German U-boat doing that.
While I haven't researched the Italian subs nor the incident you refer to I suspect what really happened to the Lulworth Hill can be explained by a review of the article at: [www.uboat.net].
Note that there is no mention of any machine gunning of the crew of that ship in Tennent, Alan J. British and Commonwealth Merchant Ship Losses to Axis Submarines 1939-1945. Stroud: Sutton Publishing, 2001. ISBN: 0-7509-2760-7. Copyright: Alan J. Tennent, 2001. Of course that doesn't mean that it didn't happen but had it been in the official records I would have expected it to be in that book.
