Hello John,
These sentences seem to me to be in Dutch, but with (orthographic-) mistakes and strange shortages.
My guess :
1. (Voucher of ?) switch of sick man
2. Makeover/acquisition of sick crew member of U69
3. Crew member got a shrapnel in his face. Wounded.
4. Deep-sea test, explosion and water flooding/breakdown. Return.
5. CO(²)-poisoning by some of the crew.
