I too had read a book on the subject so glad Ken can help. Only one instance of underwater refueling ever happened and when I find my ref notes I will advise.
For the most part hoses etc were floated across. The vulnerability of this is obvious which is why with Ultra intercepts they were all nabbed at some point or another. The Germans naturaly went to great lengths to find secret/remote locations.
You can imagine what it felt like being caught on the surface refuelling two other boats by the RN or USN aircraft.
