Hello Steve,
The incident described by your father may refer to convoy ON 127 (September 1942). HMS WITCH and ANNAPOLIS were just joining the convoy and signalling to HMCS OTTAWA when the latter distracted by their arrival was struck by a torpedo from U-91, the destroyer was finished off by a second torpedo from the same U-boat and WITCH raced to hunt for the U-boat but did not detect it and left the scene without attempting to rescue OTTAWA's survivors. Some were later rescued by HMCS ARVIDA and HMS CELANDINE but 137 lives were lost. Of course this is only a suggestion, your father my have had another incident on his mind.
