Hello David,
I cannot find the name of the Warner's CO but he did NOT survive. The senior survivor was Lt Yuile, RNR, one of the survivors rescued by HM SS D.3. Another surviving officer was Lt R. Roberts.
Interestingly Lt Yuile, who was taken aboard the sub, is adamant in his report about the identity of the sub being U-38 and not U-61.
