Iron Coffins - Both Three Times, Full Ahead!

In Cmdr. Werners' "Iron Coffins", his early Commander Ottokar Paulssen is reported as giving the order "Both Three Times Full Ahead" when escaping from destroyer escorts. How is this different from a simple "Both Full Ahead"?
Cmdr. Werner also reports firing a fan of torpedoes resulting in 3 torpedo explosions and smoke clouds on his patrol in grid square BF15 on or about April 20, 1944, yet he has no ships sink or damaged to his credit. Is that because he did not observe their sinking and the Allies did not report sunk or damaged boats?
A splendid book.
