HMS Upholder 1940 - 1942

My father was on this submarine under the command of Capt Wanklyn in WW2. Whilst refuelling in Valetta harbour, Malta there was an explosion in the torpedo motor and carbon monoxide gas escaped. Out of a team of 6 my father was the only one that survived the carbon monoxide poisoning. A family member is writing memoirs and we would like to know the date of the accident - we think it was late 1941 or early 1942. Can anyone please help?
