The text as written is a strange mix of Dutch and German (and also English if "For" is really written as "for" in the original.

Omwisseling (notice the different spelling) is Dutch for exchange of two items/people/...

Without knowing the context, I would describe the event as: An ill person was transferred for getting medical care and another person came on board as a replacement.
