HMS Fairfax

Hi all,
on 18/10/43 the US destroyer Mackenzie was involved in multiple collisions in Swansea harbor. It is stated in the Action Report that she collided first with Fairfax then with another sweeper.
My question is: I cannot find Fairfax anywhere except in one single line of Colledge Vol. I. Does anybody have any info on this vessel and possibly the identity of the other sweeper which was hit on this occasion? Mackenzie required 41 days worth of repairs so it was not just a scraping.
