Hans-Georg Fisher

On todays featured captain it had Hans-Georg Fisher. In it it states; When Fischer returned to port after his first patrol Dönitz deemed that he was unsuitable for commanding a U-boat and removed him from command. (Blair, vol 1)
From here he went on to various other duties.
My question is do we know what happened that he was removed from command.Also how rare was this in the U Boat arm. I know it happens in all armies for various reasons, but to be removed in just 1 patrol one would think he messed up bad, because they spent a lot of time and resources getting him ready to command.
