2 May 1941 (position 33.59, 12.01)
HMS Upright (Lt. R.S. Brookes, DSC, RN) tried to attack a convoy about 50 nautical miles South-East of the Kerkennah shallows. Due to an mistake in the setup of the attack it had to be broken off. Later one torpedo was fired from great range but it missed.
According to Italian sources this was probably the convoy consisting of Tilly M. Russ (1600 GRT, built 1926), Brook (1225 GRT, built 1927), Bainsizza (7933 GRT, built 1930), San Andre (?) and tug Max Behrendt escorted by the torpedo boats Generale Carlo Montanari, Clio, Centauro and Polluce and the armed merchant cruiser Ramb III, they were on passage Trapani to Tripoli.
(All times are zone -2) 1623 hours - In position 33°59'N, 12°01'E sighted smoke bearing 260°. Altered course to investigate.
1638 hours - Sighted a convoy of five merchant ships, three of about 4000 tons and two smaller ones of about 2000 tons. They were escorted by two destroyers and two aircraft. Started attack.
1652 hours - Due to a faulty attack setup by Uprights inexperienced Commanding Officer she ended up 300 yards in front of one of the escorts when she was to fire torpedoes the attack had to be broken off and Upright was taken deep.
1728 hours - Returned to periscope depth. Saw one of the merchant vessels a bit behind the remainder of the convoy. Decided to attack despite the rather great range.
1743 hours - Fired one torpedo from 6000 yards at the last ship of the Port column. It missed. (6)

The exercises had to be broken off when HMS Griffin obtained contact on a submarine which was not HMS Osiris.
HMS Osiris then immediately returned to harbour.
The destroyers conducted a short A/S hunt and then also returned to harbour. (10)

On surfacing after dark,
HMS Osiris (Lt. R.S. Brookes, DSC, RN) had an electrical malfuncion which resulted in her navigation lights buring brilliantly. The enemy must have noticed this as A/S forces began a hunt in the area. Osiris therefore shifted her patrol area to Candia. (10)

Late in the morning, HMS Osiris (Lt. R.S. Brookes, DSC, RN), was depth charged by what is thought to be an enemy destroyer / torpedo boat off Suda Bay. Lt. Brookes thought Osiris might have been detected by loops near Suda Bay. (10)

Late in the evening, HMS Osiris (Lt. R.S. Brookes, DSC, RN), suffered from a major engine defect. The starboard thrust block ran hot. (11)

Af few hours after the starboard thrust block ran hot on board HMS Osiris (Lt. R.S. Brookes, DSC, RN), the port thrust block also ran hot. Osiris was then helpless for four hours in a heavy sea, eleven miles from an enemy lee shore. This was the beginning of a long series of defects. (11)

At 0650 hours, Lt. R.S. Brookes, DSC, RN decided to abandon the patrol due to the many serious defects. HMS Osiris then set course towards Alexandria. (11)

At 2255 hours, HMS Osiris (Lt. R.S. Brookes, DSC, RN), suffered another major engine defect when her starboard main engine seized solid. By now both main moters were out of action due to flooding. Only the port main engine was now left for propulsion. (11)

HMS Clyde (Lt. R.S. Brookes, DSC, RN) attacked a submarine with two torpedoes in the central Mediterranean in position 34°52'N, 19°15'E. No hits were in fact obtained although the British thought the submarine was most probably sunk. The target mas most likely the Italian Narvalo which was on the way back to Taranto after a supply mission to Bengazi. The attack was not observed by the Italians.
(All times are zone -2) 1052 hours - When in position 34°52'N, 19°15'E received a singal from Capt. S.1 (0953/10) routing HMS Clyde to the north due to Uboat activity near her original route. Course had just been set when A/S reported HE bearing green 100°. A look on that bearing showed two small columns of brown smoke and shortly afterwrds a submarine was seen hull down. The enemy's course was estimated to be north. Started attack although the range was extreme.
1109 hours - A rain squall so HE became almost inaudible.
1123 hours - The rain cleared. Sighted the submarine bearing red 110° at a range of 8000 yards. Altered course to attack. The enemy was thought to be Italian.
1130 hours - Fired two bow torpedoes (only two were carried). The enemy again disappeared into a rain squall. Went to 80 feet.
1139 hours - Heard two explosions 16 seconds apart. Went to 38 feet to have a look through the periscope.
1143 hours - Observed an object on the horizon thought to be the stern of the submarine rising out of the water and sinking almost vertically. (21)
