With her shakedown completed USS Purdy departed from Trinidad bound for San Diego, California.
17 Mar 1945
USS Purdy arrived at Leyte, Philippines.
27 Mar 1945
USS Purdy departed from Leyte to participate in the Okinawa campaign.
1 Apr 1945
USS Purdy arrived in the Okinawa area.
12 Apr 1945
USS Purdy is heavily damaged by Japanese kamikaze aircraft of Okinawa in position 27°16'N, 127°50'E. 15 of her crew were killed and 58 were wounded. Purdy put into Kerama Retto for temporary repairs. Later she headed to Guam for further repairs.
28 May 1945
USS Purdy arrived at San Francisco for full repairs and overhaul.
