Sunk after being hit by kamikaze planes in Ormoc Bay, Leyte, Philippines in position 09º50'N, 124º55'E, 103 of the crew were killed and 152 survived though 45 were wounded which included the Commanding Officer Commander Samuel Aldo McCornock, USN.
